1) This is the animus. I think that you can load characters into wherever you want. Multiplayer is Abstergo's simulation. I could be possible that they can mix and match any locations, characters and skills they want to.
2) Not all Ottoman and Byzantine multi-player characters stayed in Constantinople for their entire life. It's possible that some could have even traveled to Italy.
